I am happy to share with you Frederiksted Health Care’s decision to transform our system of care to the Patient Centered Medical Home model and to seek formal designation as a Patient Centered Medical Home from the National Committee on Quality Assurance.  We have long believed in the importance of placing the needs of our patients at the center of our operations, and the Patient Centered Medical Home model will help us do that in a systematic way.  We will be working over the next year to improve our systems in five key areas:

  • Team-Based Care and Practice Organization
  • Knowing and Managing Our Patients
  • Patient Centered Access and Continuity
  • Care Management and Support
  • Care Coordination and Care Transitions
  • Performance Measurement and Quality Improvement
