I am happy to share with you Frederiksted Health Care’s decision to transform our system of care to the Patient Centered Medical Home model and to seek formal designation as a Patient Centered Medical Home from the National Committee on Quality Assurance.  We have long believed in the importance of placing the needs of our patients at the center of our operations, and the Patient Centered Medical Home model will help us do that in a systematic way.  We will be working over the next year to improve our systems in five key areas:

  • Team-Based Care and Practice Organization
  • Knowing and Managing Our Patients
  • Patient Centered Access and Continuity
  • Care Management and Support
  • Care Coordination and Care Transitions
  • Performance Measurement and Quality Improvement

On this page you will find links to materials that provide details on the Patient Centered Medical Home.  I hope that you will find these helpful to understand the role of the medical home.

I know that you will see positive changes in the upcoming months, and I am excited to hear your thoughts as we transform our system.  I encourage you to share feedback with us on the care that we provide through our annual patient surveys or just reach out to your medical team or myself.

Yours in health,

Masserae Sprauve Webster
